Light sensitivity,the medical term is photophobia,can range from the mildly bothersome to a symptom of a serious, sight-threatening, or even life-threatening condition. Understanding which category your symptoms fall into is the difference between monitoring at home and calling for urgent assessment.

Common, Usually Non-Urgent Causes

Migraine

Light sensitivity is one of the hallmark features of migraine. It occurs because the trigeminal nerve,which mediates pain in migraine,also has connections to the pupillary light reflex. During or after a migraine, bright light activates pain pathways even without a new headache stimulus. Dry Eye

An unstable tear film exposes the corneal nerve endings to the environment. This triggers a heightened sensitivity to light in some patients,often described as discomfort rather than true pain in bright light. Managing dry eye typically resolves this component of photophobia.

Contact Lens Overwear

Wearing contact lenses for too long causes corneal hypoxia and micro-abrasion. Both produce light sensitivity. Removing the lenses and resting the eyes usually resolves the symptoms within hours.

Conditions Requiring Urgent Assessment

Iritis / Anterior Uveitis

Inflammation of the iris and anterior uveal tract causes intense photophobia, a deep ache in or around the eye, and a small or irregular pupil. This is a condition requiring same-day assessment and treatment. Left untreated, uveitis causes permanent structural damage to the eye.

Corneal Ulcer

Bacterial, viral (herpes simplex), or fungal infection of the cornea causes severe photophobia, pain, and a white or grey opacity visible on the corneal surface. This is a sight-threatening emergency.

Acute Angle-Closure Glaucoma

A sudden, dramatic rise in intraocular pressure. Causes severe eye pain, dramatically reduced vision, halos around lights, and often nausea and vomiting. A medical emergency,call triple zero or go directly to Westmead Emergency.

Meningitis

Severe photophobia in the context of headache, fever, neck stiffness, and rash is a sign of bacterial meningitis. Call triple zero immediately.

Sudden severe photophobia with eye pain or vision change: seek urgent care.
