FL-41 lenses in stock. Many practices can theoretically order FL-41 lenses but do not keep them readily available. An optometrist who regularly works with migraine and photophobia patients will have FL-41 lenses ready to dispense without a lengthy wait, and will be able to advise on tint density based on your specific presentation.

Binocular vision assessment capability. Migraine patients frequently have co-existing binocular vision anomalies. An optometrist experienced in migraine management will assess convergence, accommodation, and ocular alignment as part of the consultation, not just refraction. These findings can change the overall management recommendation significantly.

Understanding of the photophobia mechanism. An optometrist who can explain why fluorescent lighting and screens are specifically more problematic than other light sources, and who understands the role of intrinsically photosensitive retinal ganglion cells in migraine photophobia, is one who will make a more nuanced and useful clinical recommendation.

Therapeutic endorsement. For migraine patients who also have co-existing ocular conditions (dry eye with photophobia is very common), a therapeutically endorsed optometrist can diagnose and treat these conditions independently without requiring a separate GP visit.

What Therapeutic Endorsement Means

Therapeutic endorsement is an advanced qualification held by a small proportion of Australian optometrists. It is granted by the Optometry Board of Australia (AHPRA) and permits the optometrist to diagnose and treat a wider range of ocular conditions and to prescribe Schedule 4 ocular medications independently.

For migraine patients, this matters in two specific ways. First, if a co-existing ocular condition is identified during your assessment (dry eye disease with significant photophobia, for example), it can be treated in the same appointment rather than requiring a referral. Second, the level of clinical training required for therapeutic endorsement means that therapeutically endorsed optometrists typically have a deeper grounding in anterior and posterior segment disease, which informs a more complete assessment of the migraine patient's visual system.

Therapeutic endorsement does not change the management of systemic migraine medications. Prescribing triptans and preventive migraine medications remains with GPs and neurologists. What it changes is the depth and scope of optometric care available in the one appointment.
