Is Glaucoma Hereditary? What Your Family History Means for Your Eyes

Glaucoma is one of the most strongly hereditary eye conditions. If a first-degree relative (parent, sibling, or child) has been diagnosed with glaucoma, your risk of developing the condition is approximately 10 times higher than the general population. Despite this, the vast majority of people with a family history of glaucoma are not being monitored at the appropriate frequency,often because they don't know the risk applies to them.

What Makes Glaucoma Hereditary

Primary open-angle glaucoma,the most common form,has a clear genetic component. Multiple gene variants have been identified that influence intraocular pressure regulation and optic nerve vulnerability. These variants cluster in families. The inheritance pattern is polygenic (involving multiple genes), meaning it doesn't follow a simple dominant or recessive pattern,but the family risk is real and well-established.

Your Risk Level Based on Family History

  • First-degree relative with glaucoma: 10× increased risk. Annual monitoring from age 40 (or earlier if any other risk factors are present) is strongly recommended.
  • Second-degree relative with glaucoma: Approximately 3–4× increased risk. Monitoring from age 45, or standard two-yearly screening from age 40 with additional risk factor assessment.
  • No family history: Standard population risk. Screening recommended every 2 years from age 40 via comprehensive eye examination.

Why Family History Changes What We Do at Each Appointment

When Dr Tahiri knows a patient has a family history of glaucoma, it changes the examination protocol: intraocular pressure is measured at every visit, not just when requested; OCT imaging of the optic nerve and retinal nerve fibre layer is performed annually; visual field testing is included as standard rather than optional; and any borderline IOP reading triggers additional investigation rather than a "watch and wait" approach.

Glaucoma Is Preventable,With Monitoring

This is the most important message: glaucoma causes irreversible vision loss, but it progresses slowly. In the vast majority of patients, if glaucoma is detected early and managed with appropriate treatment (eye drops, laser, or surgery), vision loss is minimal or absent. The tragedy of glaucoma is when it is detected late,when significant peripheral vision has already been lost. Annual monitoring in high-risk individuals is the single most effective intervention available.
