High Blood Pressure and Your Eyes,What a Retinal Exam Reveals

The retina is the only tissue in the body where arteries and veins can be directly observed without any surgical procedure or imaging equipment beyond an ophthalmoscope or OCT scanner. This makes retinal examination uniquely valuable for detecting systemic vascular disease,and hypertension (high blood pressure) is one of the most common conditions that reveals itself in the retinal vasculature.

What Hypertension Does to the Retinal Vessels

Normal retinal arteries are approximately two-thirds the calibre of the adjacent veins. Sustained high blood pressure causes changes to this relationship and to the vessel walls themselves, progressing through characteristic stages:

  • Early: Arterial narrowing,retinal arteries appear thinner than normal relative to veins. This is often the first sign of chronic hypertension visible on examination.
  • Moderate: Arteriovenous nicking,where arteries cross over veins, the thickened arterial wall compresses the vein beneath, causing an apparent nicking or indentation. This reflects significant arterial wall changes and is associated with increased risk of branch retinal vein occlusion.
  • Advanced: Copper and silver wiring,the arteries develop a bright, reflective appearance due to wall thickening. Further progression shows haemorrhages (flame-shaped bleeds in the nerve fibre layer), hard exudates (lipid deposits from leaking vessels), and cotton wool spots (areas of ischaemia).
  • Severe (hypertensive emergency): Papilloedema (optic disc swelling) indicates severely elevated blood pressure affecting the optic nerve. This requires emergency medical treatment.

What This Means for You

If Dr Tahiri identifies hypertensive retinopathy changes at your eye examination, she will advise you to have your blood pressure checked urgently by your GP if you are not already being managed for hypertension. In many cases, retinal changes prompt a hypertension diagnosis that was previously unknown,preventing stroke, heart attack, and kidney disease.
