
Many construction and trades workers accept chronically irritated, red, and uncomfortable eyes as "just part of the job." They are not. Occupational dry eye disease is a recognised clinical condition that can be assessed, managed, and treated,with significant improvement in quality of life and long-term eye health outcomes.
How Dust Damages the Ocular Surface
The ocular surface is a delicate mucous membrane system maintained by a complex three-layer tear film. In clean environments, the tear film replaces itself with each blink and manages normal environmental exposure effectively. In high-dust environments, the system is overwhelmed:
- Mechanical abrasion: Fine particles score the conjunctival surface with each blink, causing chronic micro-abrasion and inflammation
- Tear film contamination: Dust particles alter the composition and stability of the tear film, causing it to break up more quickly between blinks
- Increased blink rate and reflex tearing: The eye's protective response produces reflex tears,paradoxically watery eyes in a dusty environment
- Chronic conjunctivitis: Persistent particle exposure causes chronic low-grade conjunctival inflammation,the baseline "red eyes" that many outdoor workers accept as normal
Highest Risk Materials
- Concrete and cement dust: Alkaline pH is additionally irritating to the ocular surface beyond mechanical effects
- Fibreglass particles: Microscopic glass fibres cause intense irritation and are difficult to flush out
- Sawdust: Wood tannins and resins have chemical irritant properties in addition to mechanical effects
- Metal filings: From cutting and grinding,sharp-edged particles cause corneal abrasion
- Silica dust: Fine crystalline particles with particularly sharp edges at the microscopic level
Treatment and Management
Dr Tahiri assesses occupational dry eye with the same thoroughness as any other dry eye presentation: tear film quality, meibomian gland function, conjunctival and corneal surface integrity, and symptom severity. Treatment is tailored to the type and severity of damage:
- Preservative-free lubricating drops for use during and after work
