Concrete, Lime or Cement in the Eye,Why This Is a True Emergency

Of all construction-related eye injuries, alkaline chemical burns are the most serious. Concrete, cement, lime, plaster, and mortar are all highly alkaline. When they contact the eye, they do not simply irritate the surface,they penetrate through the cornea and into the interior of the eye, causing ongoing damage that continues even after the initial contact is removed.

This is categorically different from most other eye injuries. It is a time-critical emergency.

Why Alkali Is More Dangerous Than Acid

When acid contacts the eye, it causes immediate coagulation of surface proteins. This coagulation actually forms a partial barrier that slows further penetration. Counterintuitively, this means acid burns, while serious, tend to be self-limiting at the surface.

Alkali does the opposite. It causes liquefactive necrosis,it dissolves tissue rather than coagulating it, allowing it to penetrate deeper with each passing second. Concrete dust with a pH of 12–13 can penetrate to the anterior chamber of the eye within minutes of contact.

What To Do,The First 30 Seconds Are Critical

  • Irrigate immediately and continuously. Do not stop to call anyone first. Hold your eye open under the nearest source of clean water,a hose, a tap, a water bottle,and flush continuously for a minimum of 20–30 minutes.
  • Remove contact lenses if wearing them,after beginning irrigation, not before.
  • Check the pH if possible,litmus paper or pH strips on site can confirm the chemical has been sufficiently diluted (target pH 7–8).
  • Call triple zero (000) or have someone drive you to Westmead Hospital Emergency. Severe alkaline burns require emergency ophthalmology.

For Mild Exposures

Not every cement splash is a catastrophic burn. Fine dust landing briefly on a well-irrigated eye may cause surface irritation rather than a penetrating injury. After thorough irrigation, call us. We can assess the pH, examine the ocular surface under magnification, and determine whether emergency referral is needed or whether outpatient management is appropriate.

Prevention

  • Always wear AS/NZS 1337-rated eye protection when mixing, pouring, or cutting concrete and masonry products
  • Wrap-around styles provide better protection than standard spectacle-style safety glasses
  • Keep an eye wash station accessible on concrete and masonry work sites
  • If you wear prescription glasses, get prescription safety glasses rather than relying on OTG (over-glasses) protection
