
The most common question from patients who have had cataract surgery: "Can the cataract come back?" The direct answer is no,cataract surgery permanently removes the natural clouded lens and replaces it with a clear artificial intraocular lens (IOL). The original cataract cannot return because there is no longer a natural lens for it to form in.
However, up to 20–40% of patients do experience a gradual return of blurred or misty vision in the months to years following surgery,and this is so similar to the original cataract symptoms that many patients believe their cataract has returned. What is actually happening is a different condition: posterior capsule opacification.
What Is Posterior Capsule Opacification (PCO)?
During cataract surgery, the natural lens is removed but its outer shell,the lens capsule,is left in place to support the intraocular lens. In some patients, residual lens epithelial cells migrate across the back of this capsule and multiply, forming a cloudy membrane behind the IOL. This is posterior capsule opacification, sometimes called a "secondary cataract."
PCO causes exactly the same symptoms as the original cataract: blurred or foggy vision, glare around lights, and reduced visual clarity.
Treatment,YAG Laser Capsulotomy
PCO is treated with a simple, painless, in-office laser procedure called YAG laser capsulotomy. An ophthalmologist uses a laser to create a small opening in the opaque capsule behind the IOL, immediately restoring clear vision. The procedure takes 5 minutes, requires no anaesthesia beyond eye drops, and has essentially no recovery period.
