How Bulk Billing Works at an Optometrist, and How to Find One Near You in Western Sydney

What Bulk Billing Actually Means

When an optometrist bulk bills your eye test, it means they accept the Medicare rebate as full payment for the consultation. You pay nothing out of pocket. There is no gap fee, no upfront payment, and no invoice to claim back later.

Not all optometrists bulk bill. Some charge a gap fee on top of the Medicare rebate, meaning you pay the difference between their fee and what Medicare reimburses. Others do not accept Medicare at all for certain item numbers. When you book, it is always worth asking: "Do you bulk bill, and is there any gap fee for my visit?"

Who Is Eligible?

To have your eye test bulk billed, you need:

  • A valid Medicare card in your name
  • To be enrolled in Medicare (Australian citizens and most permanent residents qualify)

Temporary visa holders, overseas visitors, and some visa categories are not eligible for Medicare and therefore cannot have a bulk billed eye test. If you are unsure of your eligibility, contact Services Australia or ask us when you book.

How Often Can You Claim?

Medicare covers a standard comprehensive eye examination once every three years for most adults aged 65 and under. For adults over 65, testing can be claimed annually. Children can claim more frequently. Those with specific diagnosed conditions, including diabetes, glaucoma, and macular degeneration, may also be eligible for more frequent bulk billed testing. Your optometrist will assess your eligibility at the time of booking.

What Bulk Billing Does NOT Cover

This is the part that catches many patients by surprise. Bulk billing covers the eye test itself, nothing else. It does not cover:

  • Glasses frames or lenses
  • Contact lenses or contact lens fitting fees
  • Specialty testing not included in the standard item number
  • Prescription sunglasses

Glasses and contact lenses are covered, in part or in full, by private health fund optical extras. If you have health fund cover with optical extras, we process your claim on the spot via HICAPS. We also have no-gap glasses options for patients with optical extras cover.

Why Some Clinics Do Not Bulk Bill

Bulk billing is a choice each practice makes. Some clinics charge gap fees because they operate in high-rent commercial locations, or because their business model depends on a higher per-consultation revenue. In some cases, a clinic may not accept Medicare for certain item numbers (such as contact lens consultations) even if they bulk bill standard eye tests.

There is nothing wrong with a clinic that charges a gap fee, provided they are transparent about it. But if you are looking for a zero out-of-pocket eye test, confirm before you book.
