Blepharitis and Eyelid Hygiene,The Most Overlooked Dry Eye Treatment

Blepharitis is one of the most common eye conditions in clinical practice. It is a chronic inflammation of the eyelid margins,the strip of skin from which the eyelashes grow,and it is a leading cause of dry eye disease and ocular surface discomfort. Despite its prevalence, most patients present to us having never heard of it, and having been told their symptoms were "just dry eye" without addressing the underlying cause.

Types of Blepharitis

Anterior Blepharitis

Involves the outer front edge of the eyelid where the lashes are attached. Typically caused by Staphylococcus bacteria or Demodex mites. Symptoms include flaking of the skin at the lash base, crusting that accumulates overnight, occasional lash loss, and collarette formation around individual lashes.

Posterior Blepharitis (Meibomian Gland Dysfunction)

Involves the inner rim of the eyelid where the meibomian glands open. This is the most common form. The meibomian glands produce the oily layer of the tear film,when their secretions become thickened and waxy, the oily layer fails, causing rapid tear evaporation. This is the most common cause of evaporative dry eye disease.

Symptoms

  • Chronic burning, stinging, or gritty sensation in the eyes
  • Crusty or sticky eyelids, especially in the morning
  • Red eyelid margins
  • Intermittent blurred vision that clears with blinking
  • Eyes that water excessively (paradoxical,the eye produces reflex tears to compensate for poor tear quality)
  • Sensitivity to eye makeup, particularly mascara and liner applied to the waterline

The Eyelid Hygiene Routine

Eyelid hygiene is the cornerstone of blepharitis management. It is not optional,without it, other treatments provide only partial relief. Dr Tahiri tailors the routine to the type of blepharitis:

  • Warm compresses: Apply a warm face cloth or heated eye mask to closed eyelids for 5–10 minutes. This softens thickened meibomian gland secretions, allowing them to be expressed during the subsequent massage step.
  • Lid massage: After warm compress, gently massage the eyelid margin in a rolling motion to express the softened gland contents.
  • Lid scrubs: Use a dedicated lid scrub product or diluted baby shampoo to clean the lash bases and eyelid margin, removing accumulated debris, bacteria, and old secretions.
